Treatment Plans and Fees

• After your initial consultation, you will receive a detailed written treatment plan outlining the recommended dental procedures. The plan may change if unexpected issues arise during treatment (e.g., discovering a tooth needs a root canal after removing an old crown).
• Fees are calculated based on the specific services provided (like crowns, implants, etc.) and consider factors such as clinical time, materials, and overhead costs. The fees listed in the treatment plan are valid for 3 months from the date of the plan. Any costs from other specialists mentioned are estimates and are not included in Sid Kothari Prosthodontics fees.

Payment Policies

• Full payment for treatments is required on the day of service. Payments can be made by cash, Eftpos, Visa or MasterCard.
• A deposit may be required for new patients or larger treatment plans involving multiple appointments to secure the necessary time. Progress payments may be requested for treatments involving labs or implants, with the final payment due at the initial placement of the final prosthesis.
• Treatment may be postponed if there is an outstanding balance.

Appointment Attendance

• If you need to cancel or reschedule an appointment, you must provide at least 48 hours’ notice; otherwise, you may be charged a fee to cover the lost time.
• Appointments are specifically reserved for you and coordinated with lab work and healing times, so missed appointments affect your treatment schedule and may delay your overall plan.
• In the event that technical delays occur with lab work, Sid Kothari Prosthodontics will notify you as soon as possible and reschedule to minimize any impact on your treatment.

Health Insurance and Rebates

• If you have private health insurance, you may receive a rebate for your dental treatment. However, the rebate amount is determined by your insurer and may not match the actual fees charged by Sid Kothari Prosthodontics.
• Your relationship with your health insurer is separate from your treatment at Sid Kothari Prosthodontics. Any issues with rebates or insurance claims must be resolved directly with your insurer.

Warranty on Dental Treatment

• Prosthetic treatments are covered by a 5-year warranty against technical faults or construction defects, provided you attend annual review appointments. The warranty does not cover issues like decay from poor oral hygiene, accidental damage, or aesthetic changes requested after treatment.
• If your case involves other dental practitioners, Sid Kothari Prosthodontics is not responsible for the outcomes of their work.

Discontinuation of Treatment

• If you choose to discontinue treatment, you must pay for the services provided up to that point. Sid Kothari Prosthodontics will not be responsible for your oral health after discontinuation, and no warranty will apply to incomplete treatment.

Initial consultation

• New Patient Deposit: A $68 deposit is required to secure new patient appointments. This deposit will be applied to your account or refunded if your insurance covers the entire cost.
